Prevalence and Management of Thyroid Cancer in Nigerians Presenting with Goiter

Objective: To determine the prevalence and histological types of thyroid cancer in patients presenting with goiter in a Nigerian tertiary hospital, and to describe the mangement and treatment outcome in these patients. Methods: Consecutive patients admitted into the surgical wards of the University of Benin Teaching Hospital with goitre from January 2001 to June 2005 were studied. Provisional diagnosis was based on pre-operative clinical examination, ultra-sound of the neck, estimation of TSH, T3, T4, and FNAC when indicated, and diagnosis was based on histology report after thyroidectomy. Patients were followed up at six weeks after surgery and every six months subsequently. Results:Of 133 goiter patients, 127(95.4%) had thyroidectomy, 116(91.3%) female and 11(8.7%) male, F:M ratio of 10:1. Their ages ranged from 18 to 50 years, mean 28 years. Thirteen (10.2%) were diagnosed with thyroid cancer, 6( 46.2%) had papillary carcinoma, 5(38.5%) follicular carcinoma, 1(7.7%) mixed papillary and follicular carcinoma, and 1(7.7%) Hurthle cell carcinoma. Ten patients had subtotal, and 3 had total thyroidectomy. The weight of resected goitre ranged from 50 to 350gms, mean 148gms. Papillary carcinoma occurred in the younger patients, while follicular carcinoma was more in the older patients. No anaplastic or medullary carcinoma or lymphoma was found in the study. All 127 patients are alive and have been followed up from 14 to 57 months.
