Late Presentation with Cancer of the Prostate in a Developing Country – the Need for Education and Screening Programmes

Objective:"Cancer of the prostate (CaP) disproportionately affects the blacks than the whites, and the disease is of worse prognosis stage for stage in the former than in the latter. The study is to elucidate the peculiarities of clinical presentation of CaP in Nigeria and how it emphasizes the need for education/screening programmes."

Methods:"All confirmed cases of CaP seen over an 18 month period (September 2002 – February 2004) at the University of Benin Teaching Hospital, Benin, Nigeria were analysed for age, serum PSA, initial diagnosis, associated complications/metastases, histological grading/Gleason score, and treatment."

Results:"There were 45 confirmed cases of CaP seen during this period. The mean age was 68.5 years (median and mode was 70 years). The youngest patient was 45 years. The mean serum PSA was 45ng/ml ( range of 4 – 228ng/ml). The clinical diagnosis of CaP was correct in 39 (86.67%), with 24 (61.54%) of them presenting with one or more complications/metastases. The complications/metastases were bone metastases 15(33.33%), paraplegia/paraparesis 10(22.22%), anaemia 5(11.11%), chronic renal failure 4(8.89%), pelvic vein compression 1 and lymph node metastases involving the supraclavicular, cervical and axillary regions 5(11.11%). The histological gradings were well 11(24.44%), moderately 10(22.22%) and poorly differentiated 12(26.66%). Androgen deprivation therapy was done for 31 (68.88%) while 8 were lost to follow up. Six (13.33%) of the 45 patients were incidentally diagnosed CaP from prostatectomy/TURP specimen, with well differentiated carcinoma. Three had treatment, the other half had no treatment and no evidence of progression. During the period, 3(6.67%) of the 45 patients died."
